Community Mental Health and Wellbeing Services support people experiencing mental health challenges while they live in their community.
Community-based care means providing treatment and support in people's homes and local communities, rather than in hospital, wherever possible. Care is tailored to the needs of the person and focuses on supporting wellbeing, recovery and connection to everyday life.
Care is provided by a multidisciplinary team that may include clinicians, therapists, community engagement workers and lived experience peer workers. Each team member brings a different perspective and area of expertise to support people in working towards their goals. Lived experience peer workers draw on their own experiences of mental health challenges and recovery to provide understanding, hope and support alongside other members of the care team.
This care may include regular contact with a lead clinician who helps coordinate treatment and care, ongoing review by a treating team, connection with other community supports, access to hospital support if required, opportunities to take part in group-based programs within the community, and support from lived experience peer workers.
