What is Advance Care Planning?

Advance care planning (ACP) is about making your future healthcare wishes known in case you are too sick to speak for yourself. It makes sure those who are looking after you know what your wishes are and what is important to you for your healthcare.

For assistance with completion of an Advance Care Plan please contact your local GP and request a longer appointment.

It is recommended that you bring in a copy of your Advanced Care Plan each admission, but if you would like a copy scanned to your Bendigo Health record, please forward a copy to Health Information Services.

Advance Care Planning involves making a plan for future health and personal care should a person lose their decision-making capacity.

Advance Care Planning captures peoples’ values and wishes. It enables them to continue to influence treatment decisions, even when they can no longer actively participate.

Having the conversation and listening and documenting patient wishes about future treatment is a part of ongoing planning for future care. This is pivotal for person centred care, focusing on the needs of the person and increases the likelihood of a person’s wishes being known and respected.
